*Some insurances “Carve-Out” particular services.  This means that we must also have a contract with another company in order for certain types of services to be covered.  Usually this information is on your insurance card by listing another companies name to call for certain types of services such as behavioral/mental health services, or therapy services.  If the other company listed is not on this list, FCHC may not be considered In-Network.

**Several professionals that you may see at FCHC are not included in the bill from the hospital.  Often times these professionals are required to bill separately.  These providers may not contract with the same insurances the hospital contracts with and their bill may then be considered out of network.
