FCHC Behavioral Health

Since 1981 the professionals at the FCHC Behavioral Health have been helping clients deal with many behavioral and emotional life problems. Formally known as the FulCare, FCHC Behavioral Health offers solutions to those struggling with emotional life challenges Your mental health is just as important as your physical health -one can often affect the other. Outpatient

FCHC Building

FCHC Behavioral Health Outpatient Program moved to their new location at 495 South Shoop Avenue in Wauseon in March 2021.  The program provides comprehensive outpatient care for adults 18 and over.  Some of the Outpatient Program features include:

  • Comprehensive Psychosocial Evaluation
  • Individual Counseling
  • Interdisciplinary Treatment Approach
  • Family Counseling
  • Personalized Therapeutic Care
  • Medication Management
  • Coordination of Treatment with Family Physician or Mental Health Provider

Our outpatient staff are available Monday through Thursday 8:00 AM until 5:00 PM, Friday 8:00 AM until 4:00 PM. The Outpatient department can be reached by calling 419-330-2790.

Adult and Older Adult/Geriatric Intensive Outpatient

Care

For many years, those who sought help for support and treatment of depression, stress or anxiety had to choose between inpatient hospital care or outpatient services. To help bridge the gap between inpatient and traditional outpatient we developed a  two track Adult and Older Adult/Geriatric Intensive Outpatient Program program.  Each program is designed to provide intensive treatment while allowing the client to continue with normal daily activities.

In the Adult Intensive Outpatient Program, participants come to our unit from 8:30 AM to 11:30 AM a.m. Monday through Thursday.  Participants in the Older Adult/Geriatric Intensive Outpatient Program meet on Monday, Wednesday and Friday from noon to 3:00 PM.  Individuals participate in group and individual therapy activities and counseling sessions. In each program, members return to their home environment each day, without interrupting their family life structure. The program is voluntary and length of participation varies according to each individual’s needs.

The program provide intensive individualized treatment while allowing clients at times, to maintain normal home and work schedules. Both the adult and Older Adult/Geriatric  Intensive Outpatient Programs provide a full range of services including:

The Older Adult/Geriatric Inpatient Program is a service offered to adults age 55 and older for those suffering with severe emotional life problems. Inpatient Services are provided 24 hours a day 7 days a week in a safe comfortable atmosphere.

Patients who are admitted to the inpatient program have the assurance they are receiving effective, focused treatment. Every patient who comes into the inpatient program has a complete psychiatric evaluation by a psychiatric nurse practitioner in collaboration with a staff psychiatrist. Treatment is individualized for each patient’s needs that may include:

  • Therapeutic Milieu
  • Individual Therapy
  • Group Therapy
  • Expressive Therapy
  • Patient and Family Education
  • Family Therapy
  • Medication Management

As a patient improves, specific follow-up plans are made with each patient to assure a continuous smooth transition back into their normal environment.Care 2

Accessing Services

To find out if you or someone you know may benefit from one of our programs, call us at 419-337-8661, then select option 1 to schedule a free initial consultation. Referral for treatment can be made by individuals, family members, physicians, medical professionals, clergy, mental health professionals, employers, co-workers or friends.

Who are the Staff?

The members of the Geriatric treatment team include our psychiatrists, nurse practitioners registered nurses, social workers, other therapists and specialists as needed.

Where is the program located?

The Inpatient and IOP programs are located on the fifth floor of the Fulton County Health Center at 725 S. Shoop Avenue in Wauseon, Ohio. The FCHC Behavioral Health Outpatient Department is located at 495 South Shoop Avenue on the Northwest corner of the Fulton County Health Center campus
